First, you need documentation, so come up with a standard resume. Keep it short and simple, but include all the necessary details. In general there are 4 components to a resume which would be personal details, education and academic success, work experiences and others. Do keep in mind that you should include whatever certificates you have as proof of your successes and achievements as well. If you've got letters of recommendation or testimonials, attach that too. When it comes to job application, alter your standard resume to suit the job position you're after so that it seems more appealing. Also, don't forget to write a cover letter as well. Search the internet for pointers.
